Fedora OAI provider configuration for ETD

To disseminate ETD content via Fedora OAI provider, the following metadata settings should be used:

The configuration for dynamically generated metadata content (marcxml, etd-ms) may change before this goes into production.

OAI "about" is not currently in use.

For now, all of these are the defaults except for the query, which has been adjusted to use the isMemberOfCollection relation (rather than the default, isMemberOf).  We may create a setSpec dissemination at some point to generate the set description, but that has not been done yet.
